All Metaphase products returned to Metaphase must have an RMA (Return Material Authorization) number.
PLEASE DO NOT RETURN ITEMS WITHOUT AN RMA NUMBER PROVIDED BY METAPHASE
PLEASE CALL METAPHASE AT 215-639-8699 IF YOU DO NOT RECEIVE AN RMA NUMBER WITHIN 24 HOURS.

1. Fill out the Return Material Authorization Form in its entirety
(Excluding the RMA Number)
2. Send completed forms by Email (RMA@metaphase-tech.com) to
Metaphase Customer Service
3. Metaphase Technologies will issue an RMA number to be used for the
return.
4. Place completed RMA form in the box with returned item.*
*(With RMA Number)
5. Please write the provided RMA number on the outside of the shipping box.
6. Return the authorized item(s) per shipping instructions.
